If you need to use the Care-a-Van service, you can schedule a ride by calling: 419-696-7404 after you have scheduled your appointment. Our service hours are 8 AM to 5 PM Monday to Friday. When scheduling a ride, please have the following information ready: your name, your phone number, and the time and date of your appointment. A driver will then follow-up with you.
Please Note: As a recipient of United Way funding, the Care-a-Van is required to collect household income and Medicaid information from all clients. This information is completely confidential, and is only required for United Way reporting. Our drivers have the forms, and any information will only be used for the United Way, and will not include the names of our clients. Names are used only to accumulate all statistical data to be sure we do not duplicate individuals when reporting.
